We truly understand that many people find dentistry appointments a little stressful and every member of our office will make a special effort to ensure that your visit with us is a relaxed and positive one.

If you are a new patient, please take a moment to download, print, and fill out our Patient Registration/Health History Form and our Notice of Privacy Practices Form. If you bring these into our office on your first visit, we can get right to serving you that much more promptly.
